THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Uses and Disclosures of Protected Health Information
Cannon’s Discount Pharmacy is required by law to protect the privacy of your individual health information, known as Protected Health Information (PHI). We are also required to provide you with this Notice to explain our legal duties and privacy practices and to abide by the terms stated herein.
We may use and disclose your PHI for purposes related to treatment, payment, and healthcare operations:
- Treatment: We may use PHI to fill your prescriptions and coordinate your care, such as consulting with your physician or another healthcare provider about your medications or treatment plan.
- Payment: We may use or disclose your PHI to obtain payment from insurers, pharmacy benefit managers, or other payers. This includes activities like billing and claims processing.
- Healthcare Operations: We may use your PHI to evaluate our services, conduct staff training, perform quality assessments, and engage in other pharmacy-related administrative functions.
We maintain electronic records of your PHI and use reasonable safeguards—such as daily backups and off-site storage—to protect your information. While rare, there is a minimal risk that data loss or unauthorized access could occur due to technical issues.
We may also use your PHI to:
- Contact you with refill reminders, health-related events, immunization clinics, and treatment alternatives.
- Notify your plan sponsor of necessary information.
- Contact you for possible fundraising efforts.
We may disclose your PHI without written authorization:
- To your physician or their staff when necessary.
- To another pharmacy upon your request and consent to transfer your records.
- To business associates contracted to perform services on our behalf, who are required to safeguard your information.
- To comply with legal obligations such as worker’s compensation claims, law enforcement requests, public health reporting, or health oversight activities.
Any other use or disclosure of your PHI not described in this notice will require your written authorization. You may revoke that authorization at any time in writing.
You may request restrictions on how we use or share your PHI for treatment, payment, or healthcare operations or to certain individuals involved in your care. While we are not required to agree to every request, we will make reasonable efforts to honor them.
Your Rights Regarding Your Health Information
You have the right to:
- Inspect and copy your PHI.
- Request an amendment of your records if you believe they are incorrect.
- Receive an accounting of certain disclosures made without your authorization.
- Request confidential communications through alternate means or at alternate locations, if reasonable.
- Receive a paper copy of this Notice upon request.
We may charge a reasonable fee for copies to cover costs such as labor, postage, and supplies.
Acknowledgment, Family Disclosures, and Emergencies
You may be asked to sign a log acknowledging receipt of services and this Notice. We may disclose your PHI to family members or others involved in your care, unless you object. You may restrict such disclosures by speaking with our staff.
In emergencies or if you are incapacitated, we will use our best judgment in sharing information in your best interest. This may include allowing someone to pick up a prescription on your behalf or notifying a family member of your condition.
Changes to This Notice
Cannon’s Discount Pharmacy reserves the right to change the terms of this Notice. Any changes will apply to all PHI that we maintain. The updated Notice will be available at our pharmacy and on our website. If you have questions about this Notice or believe your privacy rights have been violated, you may contact us without fear of retaliation at the contact information below or you may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights, 200 Independence Avenue SW, Washington, DC 20201.
